Episode 1 of 3: What CQC Means by Well Led and Why It Matters
Summary
CQC asks five key questions of every service. Four are about the care itself. One is about how the service is run. This video looks at that one.
What CQC means by well led
Well led asks whether leadership, management and governance make sure people get high quality, person centred care. James breaks it into three parts:
• Leadership is direction and culture
• Management is day to day delivery
• Governance is how you know it is working
Each part does a different job. Leadership sets what the service is for. Management turns it into daily practice. Governance is the evidence that it is happening, and the early warning when it is not.
Why it matters
Safe, effective, caring and responsive are all shaped by how a service is led. When oversight is weak, problems can go unnoticed for longer. That is why well led is not a separate box to tick. It sits underneath the other four questions.
The question for every service
James sums it up in one question: could you show how the service is run, and how you would know if something was going wrong?
If the honest answer relies on one person's memory, or on policies that sit in a folder, that is the place to start.
In the news
CQC's State of Care 2025/26, published on 23 October 2026, found that around a third of care homes assessed under its current framework were rated requires improvement (31%) or inadequate (3%) for well led. Inspectors said the key concerns included leaders not having the time and support to provide oversight, and not listening to staff or acting on their feedback. CQC notes that assessments are largely risk based, so the services assessed may not represent the whole sector.
